Living Will, Durable Power of Attorney

Advance Directives

Federal law requires hospitals to ask inpatients whether they have advance directives.  Advance directives allow you to take control of choices regarding your medical future.  They include a Living Will and Healthcare Power of Attorney.

Living Will

A Living Will is a legal document you can complete that declares what your wishes are regarding the use of life-sustaining treatment if you should become unable to make decisions and you are terminally ill or permanently unconscious. 

Healthcare Power of Attorney
 
A Healthcare Power of Attorney is a legal document that authorizes another person to make healthcare decisions for you if you are unable to make informed healthcare decisions for yourself.

It is a personal choice to have either of these documents.  If you have these documents you will be asked to provide us with a copy if you are spending the night in the hospital.  If you would like further information about these forms, ask your nurse.  You may also call Social Service at CFGH at (330) 971-7162.  For your convenience, you can click here (http://www.summahealth.org/media/491/12090.pdf) to download Ohio’s Living Will and Health Care Power of Attorney.  We do not advocate you complete advanced directives on the day of surgery, but will be able to answer questions.

In his first novel The Kite Runner, and now A Thousand Splendid Suns, Khaled Hosseini writes about the Afghans caught in the middle of a seemingly endless string of wars and battles for power. Both novels paint a grim and moving picture of life in a war-torn country, and of lives lived in the face of hunger, death and a bleak future. Hosseini makes you realize that, even while bombs rain down and people are dying of hunger, people still fall in love, seek friends and, mostly, try to remain human.
